New patients

Your first visit to Apollo Spine and Pain Center takes about an hour and is focused on diagnosis, not treatment. Bring your photo ID, insurance card, imaging discs and reports, and a current medication list. Procedures are scheduled at a later visit once the source of the pain is clear.

What to expect

You will meet Dr. Rangaraj — not a rotating provider. She will ask about how the pain started, what makes it better and worse, what you have already tried, and what the pain is stopping you from doing. Then she examines you and reviews your imaging with you on the screen.

At the end of the visit you should leave with a plain-language explanation of what is likely causing your pain and a written plan. If the source is unclear, that plan may start with a diagnostic block that answers the question directly.

What to bring

  • Photo ID and your insurance card
  • Imaging discs or images (MRI, CT, X-ray) and the radiology reports
  • A current list of medications and doses, including supplements
  • Records of previous injections, surgeries or physical therapy
  • Your referral, if your plan requires one
  • A list of your questions — it is easy to forget them in the room

Forms and the patient portal

Intake forms can be completed online before your visit. If you prefer paper, arrive fifteen minutes early and our front desk will hand you the same forms on a clipboard.

Please do not send personal health information by email. Use the portal or call the office.

Costs and facility fees

Apollo is physician-owned, so there is no facility fee added on top of your office visit or procedure. Our staff verifies your benefits before any procedure is scheduled and will tell you what to expect out of pocket.
